Personal Data Protection ACT (PDPA) Notice
With reference to the Personal Data Protection Act 2010 (herein after referred to as ‘the Act’) that was enforced on 15 Nov 2013, the Chinese Recreation Club Penang (CRC) is ensuring that we are compliant with this Act. Pursuant to the requirements of the Act, we are now issuing this notice to you, detailing the manner in which we process your personal data.

| Where applicable, the CRC will process your personal data, which have been provided by you to the CRC through the following forms, and including their relevant supporting documentation: | |
| · APPLICATION FORM FOR CRC OTM MEMBERSHIP; · APPLICATION FORM FOR MEMBERSHIP (FOR ORDINARY MEMBER); · APPLICATION AS A SPORTING MEMBER; · APPLICATION FOR MEMBERSHIP (FOR SPOUSE/JUNIOR); · APPLICATION FORM FOR CHILDREN OF MEMBERS (12 YEARS AND BELOW); · CONVERSION FROM JUNIOR TO ORDINARY MEMBERSHIP; and · UPGRADING APPLICATION FORM. | |

| The CRC will be processing the following personal data which may include, but is not limited to personal data as listed below: | |
| · identity (name, age, sex, date of birth, NRIC number / birth certificate number, race and nationality, marital status, birth certificate information and signature); · contact information (address, home and office phone number, mobile number and email address); · employment and educational information (occupation, educational level, designation, company name and address, length of service, name of employer and his contact number); · family details (spouse’s details and children’s details); · personal interests; · name and membership number of proposer and seconder given in application; and · photographs given to the CRC and/or taken at official club events and functions. | |

| Your information is necessary to us. Unless otherwise stated, all information requested by CRC is obligatory. If you do not provide and/or allow us to process all the obligatory information as requested, we will not be able to keep complete information about you, thus affecting our ability to accomplish the above stated purposes. | |
| 2. | USAGE OF DATA |
| The CRC will use and/or process the personal data for, amongst others, the following reasons: | |
| · Facilitating your membership with the CRC; · For contact and communication purposes, which includes sending notices (including notices informing you of our Annual General Meetings and Extraordinary General Meetings), reminders, letters, forms, annual reports and newsletters; · To invite and inform you of events and functions; · To administer and provide for any special benefits related to your membership; and · When you attend the CRC’s events, to publish your name and photographs in announcements, newsletters. | |

| Where you have consented, we will use your personal information to communicate with you on promotions and events: | |

| The CRC may disclose your personal data to the following parties (including those overseas): | |
| · Agents and service providers who provide services relating to the purposes for which the personal data is collected, including data processing services; · Any person, who is under a duty of confidentiality who has undertaken the responsibility to keep such data confidential; · Government authorities or agencies, when required; · Other parties, in respect of whom you have consented to the disclosure of your personal data. | |

| We do not disclose your personal data to unaffiliated third parties without your consent.
